• Written and signed statement by the driver/witness/claimant
• Copy of driver’s license.
• Copy of logbook.
• Police inspection report (IOV)
• Complete police report (where necessary)
• At least three repair quotation from two different sources
• Payment receipts / Towing receipts (if applicable)
• Medical reports (If third party involved)
• Postmortem / death certificate
• Third party claim letter
• Proof of ownership / Purchase details and agreement/ Original logbook (in case of theft or total loss) and identification of the owner (If applicable)
• Car keys (in case of theft or total loss)
• Bank signed transfer forms (Where applicable)
• Letter authorizing transfer (Company vehicles – Where applicable)
• Final bills & purchase receipts of repair where the repair costs are within authorized repair limits.
